WAIVER & RELEASE FORM

Because physical exercise can be strenuous and subject to risk of serious injury, LASE urges you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity including but not limited to soccer, soccer drills, conditioning, speed and agility work.

You (each member, guest, and all participating family members including minors for whom you are the parent/guardian) agree that if you engage in any physical exercise or activity, or use any LASE amenity on or off the premises, including any sponsored LASE event, you do so entirely at your own risk.

Any recommendation for changes in diet including the use of food supplements, weight reduction, and/or bodybuilding enhancement products are entirely your responsibility and you should consult a physician prior to undergoing any dietary or food supplement changes.

You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death. LASE is not responsible for any loss or theft of personal property.

This waiver and release of liability includes, without limitation, all injuries which may occur as a result of:

(a) your use of all amenities and equipment in the facility and your participation in any activity, class, program, personal training, or instruction;
(b) the sudden and unforeseen malfunctioning of any equipment;
(c) instruction, training, supervision, or dietary recommendations provided by LASE;
(d) slipping, tripping, bumping, and/or falling while at the facility or on the premises, including adjacent sidewalks and parking areas;
(e) participation in any classes, clinics, camps, tournaments, or events organized by LASE at off-site locations.

You acknowledge that you have carefully read this waiver and release and fully understand that it is a release of liability. You expressly agree to release and discharge LASE, its owners, directors, officers, employees, coaches, trainers, agents, representatives, affiliates, successors, and assigns from any and all claims or causes of action, and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against LASE for personal injury, illness, death, or property damage.

To the extent that Florida statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of LASE, its agents, and employees.

If any portion of this release from liability is deemed by a court of competent jurisdiction to be invalid, the remainder shall remain in full force and effect and the offending provision or provisions shall be severed.

By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

Media Release Authorization

I grant LASE and its legal representatives the irrevocable right and unrestricted permission to use and publish photographs or video images of myself and/or my child, or in which I/my child may be included, for any lawful purpose including but not limited to website use, social media, promotional materials, editorial publications, marketing, advertising, and print or digital media.

This grant includes the right to modify and retouch the images at the discretion of LASE.

I understand that the circulation of such materials may be worldwide and that there will be no compensation to me for this use. Furthermore, I understand that I will not be given the opportunity to inspect or approve the finished products or advertising copy that may be used in connection therewith.

In granting this permission to LASE and its legal representatives, I fully and without limitation release it from any liability that may arise from the use of such images.
